Survey's without ActivDevices

Hi,
I am developing a flipchart for an exhibit and I need to create a survey resource within the flipchart. Is it possible to make a survey that stores the results into a database that I can use with and without the activ devices.

You may want to use a web-based survey tool. Do you want them to answer multiple choice type question or free response?

I've heard of surveymonkey and zoomerang.
